⛓️ 身体拘束は、ほとんどの薬剤よりも多くのルールがあり、より短いリードで管理される医療的介入です。PRN指示がひとつ、サイドレールへの結びつけがひとつ、確認の見落としがひとつあれば、その問題に不合格となるだけでなく、患者を傷つける可能性もあります。

🧠 なぜ重要か
身体拘束は、より制限の少ない手段が効果を失ったあとの最後の手段（LAST RESORT）です：見当識を促す関わり、付き添い、ベッドアラーム、患者をナースステーションの近くへ移すこと、疼痛や興奮の原因への対処がそれにあたります。医師の指示（ORDER）が必要です：緊急時には看護師が身体拘束を行い、その直後に指示を得なければならず、暴力的または自傷的な行動に対する身体拘束では、一時間以内に対面評価も必要です。身体拘束のPRN指示は決して許可されません。暴力的または自傷的な行動に対する指示は、成人では4時間ごと、9歳から17歳では2時間ごと、9歳未満の子どもでは一時間ごとに更新しなければならず、非暴力的な医療上の身体拘束の指示は、病院の方針に従って、通常は暦日ごとに更新します。身体拘束はクイックリリースの結び方でベッドフレーム（BED FRAME）に結び、決してサイドレールには結ばず、拘束具と患者の皮膚の間に指二本分の余裕があることを確認します。方針に従い、循環、皮膚、感覚を頻繁に（通常は15分から30分ごとに）確認し、少なくとも2時間ごとに拘束を解いて体位を変え、水分摂取と排泄を促し、行動、試した代替手段、患者の反応、そして各確認内容を記録します。行動が落ち着いたら、直ちに身体拘束を解除します。
Restraints are a LAST RESORT after less restrictive measures fail: reorientation, a sitter, a bed alarm, moving the client near the station, treating pain or the cause of agitation. A provider's ORDER is required; in an emergency the nurse may apply restraints and must obtain the order immediately afterward, and restraint for violent or self-destructive behavior also needs a face-to-face evaluation within one hour. PRN restraint orders are never allowed. Orders for violent or self-destructive behavior must be renewed every 4 hours for adults, every 2 hours for ages 9 to 17 and every hour for children under 9; orders for non-violent medical restraints are renewed as hospital policy sets, commonly every calendar day. Tie restraints with a QUICK-RELEASE knot to the BED FRAME, never the side rails, and make sure two fingers fit between the restraint and the client's skin. Check circulation, skin and sensation often, commonly every 15 to 30 minutes per policy; release and reposition at least every 2 hours, offer fluids and toileting, and document the behavior, alternatives tried, the client's response and each check. Remove the restraint as soon as the behavior resolves.

🛏️ A confused client pulls at his IV. The nurse tries covering the site with a sleeve, moves the IV pump out of view, calls a family member to sit, and asks the provider to review the sedating medication. None of that is a restraint. Less restrictive steps like these should be tried first, and the ones tried should be documented, before a mitt or wrist restraint is used.

An acceptable restraint order names the type of restraint and the specific behavior it addresses, is time-limited and is never PRN. For non-violent medical restraints, the time limit is the renewal interval set by hospital policy, commonly each calendar day, so 'renew per policy' makes the order time-limited. Orders that are PRN or 'as needed', or tied to a condition such as 'whenever the family leaves the room', are never acceptable, and stating '24 hours' does not make an 'as needed' order acceptable.
