RACE, PASS and the Hazards Hiding in Plain Sight
🔥 Smoke in the hallway. Oxygen running in three rooms. A client who cannot walk. Your first move is not the extinguisher, and the exam knows most people grab it anyway.
RACE in order: rescue, alarm, confine, extinguish or evacuate. PASS for the extinguisher. People out of danger before anything else.
In a fire, RACE: RESCUE anyone in immediate danger, ACTIVATE the alarm, CONFINE the fire by closing doors and windows and shutting off oxygen as directed, and EXTINGUISH if small or EVACUATE, moving the most mobile clients first (ambulatory, then wheelchair, then bedbound) horizontally away from the fire before considering stairs. Use an extinguisher with PASS: PULL the pin, AIM at the base, SQUEEZE the handle, SWEEP side to side. Never use elevators. OXYGEN supports combustion: no smoking or open flames, no petroleum-based products near oxygen, and 'oxygen in use' signs. ELECTRICAL safety: remove equipment with frayed cords or that gives a shock, use grounded three-prong plugs, and keep electrical devices away from water. HAZARDOUS SPILLS: isolate the area, consult the safety data sheet, put on the appropriate PPE, use the spill kit and report the spill per facility policy. For a hazardous exposure, remove people from immediate danger first, then contain, then report. For a violent visitor, keep yourself and clients at a safe distance and call security. For a telephoned bomb threat, keep the caller talking and note every detail, notify security or police, touch nothing suspicious, and evacuate only when incident command directs.
🚨 A wastebasket fire starts in a client's room. The nurse wheels the client into the hall, pulls the alarm, closes the door, and only then returns with the extinguisher for a fire that is still small. Reverse that order and the nurse is fighting a fire while a client sits in the smoke.
(Evacuate)Rescue, Alarm, Confine, Extinguish or Evacuate
(Sweep)Pull, Aim at the base, Squeeze, Sweep
Move ambulatory clients first in an evacuation; never use elevators
RACE: Rescue(救出)、Alarm(警報)、Confine(封じ込める)、Extinguish(消火する)またはEvacuate(避難する)という、火災を発見したときの行動の順序です。
PASS: Pull(ピンを抜く)、Aim(狙う)、Squeeze(ハンドルを握る)、Sweep(左右に掃くように動かす)という、消火器の使い方です。
RACE(火事への対応) (RACE (fire response)): Rescue anyone in danger(危険にさらされている人を救出する)、Alarm (pull it and call)(警報を鳴らして知らせる)、Confine the fire by closing doors(ドアを閉めて火を閉じ込める)、Extinguish or Evacuate(消火するか、避難する)という、火事を見つけたときの行動の順序です。
PASS(消火器) (PASS (fire extinguisher)): Pull the pin(ピンを抜く)、Aim at the base of the fire(火の根もとを狙う)、Squeeze the handle(ハンドルを握る)、Sweep side to side(左右に振る)という、RACEを始めた後の消火器の使い方です。
A fire is discovered in a client's room. What is the nurse's first action?
Use the fire extinguisher
Activate the fire alarm
Remove the client from the room
Close all doors on the unit
Rescue comes first in RACE. Get the person out of immediate danger, then activate the alarm.
Clients in immediate danger have already been removed from the room where the fire started. During the horizontal evacuation of the rest of the unit, which clients are moved first?
Bedbound clients
Ambulatory clients
Clients closest to the fire regardless of mobility
Clients on ventilators
Anyone in immediate danger was already rescued (the R in RACE). In the evacuation itself, ambulatory clients go first: they move quickly with minimal staff, freeing resources for those who need more help.
A client's family brings a scented wax candle to light and a tube of petroleum jelly for dry lips while the client is on oxygen. Which item is a safety concern?
Only the candle
Only the petroleum jelly, because it is flammable on the skin
Both: flames and petroleum products are oxygen hazards
Neither
Oxygen supports combustion. Open flames and petroleum-based products are both prohibited; use water-based lip balm.
