Croup, Epiglotit ak Lam Lang Ou Pa Janm Itilize

Croup, Epiglottitis and the Tongue Blade You Never Use

🐕 Yon tous ki sonnen tankou japman a 2 è dimaten se croup, epi yon dòz dexamethasone trete li. Yon timoun ki an silans, ki ap bave, ki pè anpil epi ki chita panche devan gen epiglotit, epi lam lang yon enfimyè ki gen bon entansyon lonje men pran ka fèmen pasaj lè a pou tout tan.

🐕 A barking cough at 2 a.m. is croup, and a dose of dexamethasone treats it. A silent, drooling, terrified child sitting forward is epiglottitis, and the tongue blade a well-meaning nurse reaches for can close the airway for good.

💡 Ide prensipal

Croup la sonnen tankou japman epi li jwenn yon esteroyid, plis epinephrine an nebilizasyon pou estridor an repo. Epiglotit la bave, chita panche devan epi li jwenn yon ekip pasaj lè, janm yon lam lang. RSV jwenn aspirasyon ak prekosyon kontak. CF jwenn anzim ak chak bouche.

Croup barks and gets a steroid, plus nebulized epinephrine for stridor at rest. Epiglottitis drools, sits forward and gets an airway team, never a tongue blade. RSV gets suction and contact precautions. CF gets enzymes with every bite.

🧠 Poukisa sa enpòtan

CROUP (larengotrakeobwonchit): viral, ak yon tous ki sonnen tankou japman, estridor enspiratwa, vwa anwe ak yon ti lafyèv, pi mal lannwit; trete ak yon kortikosteroyid (dexamethasone oral) epi, pou estridor an repo, epinephrine rasemik an nebilizasyon, pandan w kenbe timoun nan kalm ak paran an; bwouya fre oswa lè fre lannwit ka rekonfòte timoun nan men li pa trete croup la. EPIGLOTTITIS: bakteri (klasikman Haemophilus influenzae type b, kounye a ra gras ak vaksinasyon), ak gwo lafyèv sibit, kat D yo (drooling, dysphagia, dysphonia, distress), yon pozisyon tripod epi pa gen tous; PA egzaminen gòj la, pa itilize lam lang epi pa kouche timoun nan plat; kenbe timoun nan kalm, dwat epi ak paran an, epi prepare w pou entibasyon ijans ak ekip pasaj lè a prezan. BRONCHIOLITIS (anjeneral RSV): sifleman, takipne, narin ki louvri ak retraksyon kay tibebe; swen sipò ak aspirasyon nen anvan manje, idratasyon, oksijèn jan sa nesesè, epi prekosyon kontak. PERTUSSIS: tous paroksistik ak yon son enspiratwa ki rele whoop, prekosyon goutlèt, antibyotik makrolid. CYSTIC FIBROSIS: sekresyon epè, po sale, steatorrhea ak reta nan kwasans; fizyoterapi pwatrin, anzim pankreyatik ak chak repa ak chak kolasyon, rejim ki rich an kalori ak pwoteyin ak vitamin ki solib nan grès, epi sèl anplis nan tan cho. FOREIGN BODY: tous oswa estridor sibit kay yon ti timoun; si timoun nan pa ka touse, kriye oswa pale, bay sik repete 5 kou nan do ak 5 pouse nan pwatrin anba laj 1 an, oswa 5 kou nan do ak 5 pouse nan vant pou timoun ki pi gran.

CROUP (laryngotracheobronchitis): viral, with a barking cough, inspiratory stridor, hoarseness and low-grade fever, worse at night; treat with a corticosteroid (oral dexamethasone) and, for stridor at rest, nebulized racemic epinephrine, keeping the child calm with the parent; cool mist or cool night air may comfort the child but does not treat croup. EPIGLOTTITIS: bacterial (classically Haemophilus influenzae type b, now rare with vaccination), with sudden high fever, the four Ds (drooling, dysphagia, dysphonia, distress), a tripod position and no cough; DO NOT examine the throat, use a tongue blade or lay the child flat; keep the child calm, upright and with the parent, and prepare for emergency intubation with the airway team present. BRONCHIOLITIS (usually RSV): wheezing, tachypnea, nasal flaring and retractions in infants; supportive care with nasal suctioning before feeds, hydration, oxygen as needed, and contact precautions. PERTUSSIS: paroxysmal coughing with an inspiratory whoop, droplet precautions, macrolide antibiotics. CYSTIC FIBROSIS: thick secretions, salty skin, steatorrhea and failure to thrive; chest physiotherapy, pancreatic enzymes with every meal and snack, high-calorie high-protein diet with fat-soluble vitamins, and added salt in hot weather. FOREIGN BODY: sudden coughing or stridor in a toddler; if the child cannot cough, cry or speak, give repeated cycles of 5 back blows and 5 chest thrusts under age 1, or 5 back blows and 5 abdominal thrusts for older children.

🏥 Nan lavi reyèl

🚫 Yon timoun 4 an rive ak lafyèv, li ap bave, li panche devan, li refize vale. Yon enfimyè etidyan pran yon ti flach ak yon abeslang pou li gade gòj la. Enfimyè responsab la rete li, li kenbe timoun nan sou jenou manman an, epi li rele anestezi. Wè epiglot la deyò sal operasyon an ka deklanche yon obstriksyon konplè. Bon repons egzamen an se sa ki pa fè anyen nan gòj la.

🚫 A 4-year-old arrives febrile, drooling, leaning forward, refusing to swallow. A student nurse picks up a penlight and tongue depressor to look at the throat. The charge nurse stops her, keeps the child in the mother's lap, and calls anesthesia. Visualizing the epiglottis outside the operating room can trigger complete obstruction. The exam's correct answer is the one that does nothing to the throat.

📌 Sa pou retni

  • Croup: tous ki sonnen tankou japman, estridor lannwit; dexamethasone, plis epinephrine rasemik pou estridor an repoCroup: barking cough, stridor at night; dexamethasone, plus racemic epinephrine for stridor at rest
  • Epiglotit: bave, tripod, pa gen tous: pa gen egzamen gòj, prepare pou entibasyonEpiglottitis: drooling, tripod, no cough: no throat exam, prepare for intubation
  • RSV: aspirasyon anvan manje, prekosyon kontak; CF: anzim ak chak repaRSV: suction before feeds, contact precautions; CF: enzymes with every meal

📖 Tèm nan leson sa a

Croup: Yon enfeksyon viral pasaj lè nan jèn timoun ak yon tous ki sonnen tankou jape ak stridor, yo trete l ak dexamethasone epi, pou stridor lè yo repoze, epinephrine nebulize.

Epiglotit (Epiglottitis): Yon anfle bakteri nan epiglot la ak bave ak yon pozisyon tripod; pa janm egzamine gòj la.

✅ Teste tèt ou

Yon timoun yo sispèk gen epiglotit ap bave epi li chita dwat panche devan. Ki aksyon yon enfimyè ki mande entèvansyon imedya?A child with suspected epiglottitis is drooling and sitting upright leaning forward. Which action by a nurse requires immediate intervention?
  1. Enspekte gòj la ak yon lam langInspecting the throat with a tongue blade
  2. Rele ekip pasaj lè a pou vin bò kabann nanCalling for the airway team to come to the bedside
  3. Kenbe chanm nan an silans epi ranvwaye pwosedi ki pa ijan yoKeeping the room quiet and deferring nonurgent procedures
  4. Kenbe timoun nan dwat sou jenou paran anKeeping the child upright in the parent's lap

Repons: A · Enspekte gòj la ak yon lam lang

Egzamen gòj la ka deklanche yon obstriksyon konplè nan pasaj lè a; pèsonèl kalifye sekirize pasaj lè a nan yon anviwònman kontwole. Kenbe timoun nan dwat sou jenou paran an se bon swen an: pozisyon an fasilite respirasyon, epi rete kalm limite kriye ki ka agrave obstriksyon an.

Throat examination can precipitate complete airway obstruction; the airway is secured by skilled staff in a controlled setting. Keeping the child upright in the parent's lap is correct care: the position eases breathing, and staying calm limits crying that can worsen the obstruction.

Ki entèvansyon ki apwopriye pou yon ti timoun ki gen croup ak yon ti estridor lakay?Which intervention is appropriate for a toddler with croup and mild stridor at home?
  1. Kenbe timoun nan kalm epi dwatKeep the child calm and upright
  2. Ankouraje jwèt ki anime anpil pou dekole sekresyon yoEncourage vigorous play to loosen secretions
  3. Bay medikaman kont tous san preskripsyonGive over-the-counter cough suppressants
  4. Kouche timoun nan plat sou doPlace the child flat on the back

Repons: A · Kenbe timoun nan kalm epi dwat

Kalm epi dwat fasilite respirasyon epi bese bezwen oksijèn, epi founisè a anjeneral preskri yon dòz dexamethasone oral; estridor an repo mande swen ijan. Bwouya fre ka rekonfòte men li pa trete croup la. Medikaman kont tous pa rekòmande pou jenn timoun, kouche plat agrave respirasyon, epi jwèt ki anime anpil ogmante bezwen oksijèn.

Calm and upright eases breathing and lowers oxygen demand, and the provider usually prescribes one dose of oral dexamethasone; stridor at rest needs urgent care. Cool mist may comfort but does not treat croup. Cough suppressants are not recommended for young children, lying flat worsens breathing, and vigorous play raises oxygen demand.

Yon timoun ki gen cystic fibrosis prèt pou manje midi. Ki sa enfimyè a ta dwe asire?A child with cystic fibrosis is about to eat lunch. What should the nurse ensure?
  1. Timoun nan resevwa anzim pankreyatik ak repa aThe child receives pancreatic enzymes with the meal
  2. Repa a fèb an grès ak kaloriThe meal is low in fat and calories
  3. Timoun nan evite sèlThe child avoids salt
  4. Fizyoterapi pwatrin fèt imedyatman apre manjeChest physiotherapy is done immediately after eating

Repons: A · Timoun nan resevwa anzim pankreyatik ak repa a

Anzim ak chak repa ak kolasyon pèmèt absòpsyon grès ak pwoteyin; rejim CF yo rich an kalori, grès ak sèl, epi fizyoterapi pwatrin fèt anvan repa yo.

Enzymes with every meal and snack allow fat and protein absorption; CF diets are high in calories, fat and salt, and chest physiotherapy is done before meals.
