Tamponade, Tension Pneumothorax ak Autonomic Dysreflexia

Tamponade, Tension Pneumothorax and Autonomic Dysreflexia

🫀 Son kè ki toufe, venn nan kou ki gonfle ak yon tansyon k ap bese. Twa rezilta, yon non, ak yon zegwi nan pericardium si w rele l ase vit. Egzamen an renmen triyad paske yo ka teste yo.

🫀 Muffled heart sounds, bulging neck veins and a falling blood pressure. Three findings, one name, and a needle in the pericardium if you call it fast enough. The exam loves triads because they are testable.

💡 Ide prensipal

Beck's triad vle di tamponade. Devyasyon tracheal ak breath sounds absan vle di tension pneumothorax. Doulè tèt ak hypertension nan yon blesi spinal wo vle di dysreflexia: chita dwat epi vide vesi a. Anaphylaxis: epinephrine anvan.

Beck's triad means tamponade. Tracheal deviation with absent breath sounds means tension pneumothorax. Headache and hypertension in a high spinal injury means dysreflexia: sit up and empty the bladder. Anaphylaxis: epinephrine first.

🧠 Poukisa sa enpòtan

CARDIAC TAMPONADE, likid ki konprese kè a apre chòk nan pwatrin, operasyon kè oswa pericarditis, pwodui BECK'S TRIAD: hypotension, venn nan kou ki dilate ak son kè ki toufe, souvan ak yon pulse pressure k ap vin pi etwat ak pulsus paradoxus; trete ak pericardiocentesis ijans. TENSION PNEUMOTHORAX, lè ki bloke anba presyon nan espas pleural la, parèt ak gwo detrès respiratwa, breath sounds absan sou yon bò, devyasyon tracheal KI ALE LOEN bò ki afekte a, venn nan kou ki dilate ak hypotension; trete ak needle decompression imedya apre sa yon chest tube. AUTONOMIC DYSREFLEXIA afekte pasyan ki gen blesi spinal cord nan T6 oswa pi wo: yon stimulus danjere anba blesi a, pi souvan yon vesi plen, apre sa bowel impaction oswa presyon sou po, deklanche yon hypertension grav toudenkou ak yon doulè tèt k ap bat, woujè ak swe pi wo pase blesi a, pallor ak goosebumps anba, ak bradycardia. Chita pasyan an dwat imedyatman, dekole rad sere, jwenn epi retire kòz la (tcheke catheter la, vesi, entesten ak po), kontwole tansyon chak kèk minit epi bay antihypertensive yo te bay lòd la si l pa rezoud. ANAPHYLAXIS: hives, anfleman, wheezing, stridor ak hypotension nan kèk minit apre yon deklanchè; bay epinephrine intramuscular anvan, apre sa oksijèn, likid ak sipò vwa respiratwa; antihistamines ak steroids se adjuncts, pa ranplasan.

CARDIAC TAMPONADE, fluid compressing the heart after chest trauma, cardiac surgery or pericarditis, produces BECK'S TRIAD: hypotension, distended neck veins and muffled heart sounds, often with a narrowing pulse pressure and pulsus paradoxus; treat with emergency pericardiocentesis. TENSION PNEUMOTHORAX, air trapped under pressure in the pleural space, shows as severe respiratory distress, absent breath sounds on one side, tracheal deviation AWAY from the affected side, distended neck veins and hypotension; treat with immediate needle decompression then a chest tube. AUTONOMIC DYSREFLEXIA affects clients with spinal cord injuries at or above T6: a noxious stimulus below the injury, most often a full bladder, then bowel impaction or skin pressure, triggers a sudden severe hypertension with a pounding headache, flushing and sweating above the injury, pallor and goosebumps below, and bradycardia. Sit the client upright immediately, loosen tight clothing, find and remove the cause (check the catheter, bladder, bowel and skin), monitor blood pressure every few minutes and give the ordered antihypertensive if it does not resolve. ANAPHYLAXIS: hives, swelling, wheezing, stridor and hypotension within minutes of a trigger; give intramuscular epinephrine first, then oxygen, fluids and airway support; antihistamines and steroids are adjuncts, not substitutes.

🏥 Nan lavi reyèl

🚽 Yon pasyan ki gen yon blesi C6 toudenkou gen yon tansyon 210/120, yon doulè tèt k ap bat ak yon figi wouj. Enfimyè a chita l dwat, li tcheke urinary catheter la epi li jwenn li pliye ak 800 mL ki bloke. Lè li dwate tib la, vesi a vide epi presyon an bese nan kèk minit. Yo pa t bezwen okenn medikaman, paske yo te retire kòz la. Egzamen an vle lòd sa a: pozisyon, kòz, apre sa medikaman.

🚽 A client with a C6 injury suddenly has a blood pressure of 210/120, a pounding headache and a flushed face. The nurse sits him upright, checks the urinary catheter and finds it kinked with 800 mL backed up. Straightening the tubing drains the bladder and the pressure falls within minutes. No drug was needed, because the cause was removed. The exam wants that order: position, cause, then medication.

📌 Sa pou retni

  • Tamponade: hypotension, JVD, son kè ki toufe: pericardiocentesisTamponade: hypotension, JVD, muffled heart sounds: pericardiocentesis
  • Tension pneumothorax: devyasyon tracheal ki ale lwen, breath sounds absan: needle decompressionTension pneumothorax: tracheal deviation away, absent breath sounds: needle decompression
  • Dysreflexia: chita dwat, jwenn stimulus la (vesi anvan), apre sa antihypertensives; anaphylaxis: epinephrine IMDysreflexia: sit up, find the stimulus (bladder first), then antihypertensives; anaphylaxis: IM epinephrine

📖 Tèm nan leson sa a

Triyad Beck (Beck's triad): Ipotansyon, venn kou ki gonfle ak son kè ki toufe: siy tanponad kadyak yo.

Pneumotoraks anba tansyon (Tension pneumothorax): Lè ki bloke anba presyon nan pwatrin lan ki deplase trachea a nan lòt bò a epi ki kraze sikilasyon an; dekonpresyon ak zegwi soulaje l.

Disrefleksi otonòm (Autonomic dysreflexia): Yon ipètansyon danjere sibit ak maltèt nan blesi mwal epinyè nan T6 oswa pi wo, abityèlman yon vesi plen deklanche l.

✅ Teste tèt ou

Yon pasyan ki gen yon blesi spinal cord T4 fè yon gwo doulè tèt, yon tansyon 200/110 ak swe pi wo pase pwatrin nan. Ki premye aksyon enfimyè a?A client with a T4 spinal cord injury develops a severe headache, a blood pressure of 200/110 and sweating above the chest. What is the nurse's first action?
  1. Rele founisè a anvan evalyasyon anCall the provider before assessing
  2. Kouche pasyan an plat epi leve janm li yoLay the client flat and elevate the legs
  3. Chita pasyan an dwat epi tcheke pou yon vesi ki dilateSit the client upright and check for a distended bladder
  4. Bay opioid PRN a pou doulè tèt laAdminister the PRN opioid for the headache

Repons: C · Chita pasyan an dwat epi tcheke pou yon vesi ki dilate

Chita dwat bese tansyon imedyatman; yon vesi plen se deklanchè ki pi komen autonomic dysreflexia. Kouche pasyan an plat ak janm yo leve se yon repons pou tansyon ba epi li ta pouse presyon sa a pi wo toujou.

Sitting upright lowers blood pressure immediately; a full bladder is the most common trigger of autonomic dysreflexia. Laying the client flat with the legs raised is a response to low blood pressure and would push this pressure even higher.

Apre chòk nan pwatrin, yon pasyan gen hypotension, venn nan kou ki dilate ak son kè ki toufe. Ki konplikasyon enfimyè a ta dwe sispèk?After chest trauma, a client has hypotension, distended neck veins and muffled heart sounds. Which complication should the nurse suspect?
  1. Cardiac tamponade
  2. Hemorrhagic shock
  3. Tension pneumothorax
  4. Pulmonary embolism

Repons: A · Cardiac tamponade

Beck's triad is the classic sign of cardiac tamponade, which requires emergency pericardiocentesis. Tension pneumothorax can also cause hypotension and distended neck veins, but it brings absent breath sounds and tracheal shift, not muffled heart sounds.

Yon pasyan k ap resevwa yon nouvo antibyotik IV fè hives, anfleman nan figi ak wheezing. Ki medikaman enfimyè a ta dwe bay anvan?A client receiving a new IV antibiotic develops hives, facial swelling and wheezing. Which medication should the nurse administer first?
  1. Diphenhydramine IV
  2. Epinephrine IM
  3. Methylprednisolone IV
  4. Albuterol nebulizer

Repons: B · Epinephrine IM

Epinephrine intramuscular se tretman premye liy pou anaphylaxis; antihistamines, steroids ak bronchodilators se adjuncts.

Intramuscular epinephrine is the first-line treatment for anaphylaxis; antihistamines, steroids and bronchodilators are adjuncts.
