Compartment Syndrome ak Fat Embolism: Doulè ak Petechiae

Compartment Syndrome and Fat Embolism: Pain and Petechiae

🦴 Yon janm nan plat fè mal pi plis chak èdtan epi medikaman doulè a sispann travay. Etidyan liv lekòl la leve l pi wo. Egzamen an, ak chirijyen an, vle kontrè a.

🦴 A casted leg hurts more every hour and the pain medication stops working. The textbook student elevates it higher. The exam, and the surgeon, want the opposite.

💡 Ide prensipal

Compartment syndrome: doulè san soulajman ak doulè ak passive stretch vini anvan; dekole plat la, kenbe manm nan nan nivo kè a, rele. Fat embolism: dyspnea, konfizyon ak petechiae sou pwatrin 1 a 3 jou apre yon frakti long-bone.

Compartment syndrome: unrelenting pain and pain on passive stretch come first; loosen the cast, keep the limb at heart level, call. Fat embolism: dyspnea, confusion and chest petechiae 1 to 3 days after a long-bone fracture.

🧠 Poukisa sa enpòtan

COMPARTMENT SYNDROME se presyon k ap monte anndan yon konpatiman misk, anndan yon plat oswa apre yon blesi kraze oswa yon boule, ki koupe perfusion an. Siy ki pi bonè ak pi fyab yo se DOULÈ ki pa pwopòsyonèl ak blesi a epi opioids pa soulaje l ak DOULÈ AK PASSIVE STRETCH dwèt men yo oswa dwèt pye yo, apre sa paresthesia ak anfleman tansyone; pallor, paralysis ak pèt batman yo TA epi yo vle di tisi a deja ap mouri. Aji nan kèk èdtan: dekole oswa bivalve plat la oswa pansman an, kenbe manm nan nan NIVO KÈ a (leve l pi wo pase kè a bese presyon atèryèl ki antre nan konpatiman an), pa aplike glas, epi avize founisè a imedyatman; tretman an se yon fasciotomy. FAT EMBOLISM syndrome swiv frakti long-bone ak frakti pelvic, anjeneral nan 24 a 72 èdtan: dyspnea toudenkou, hypoxemia, konfizyon oswa ajitasyon, tachycardia, lafyèv ak yon gratèl PETECHIAL sou pwatrin nan, axilla ak conjunctiva; trete ak oksijèn, sipò, ak immobilization bonè frakti yo pou anpeche l. Verifikasyon NEUROVASCULAR woutin (6 Ps yo: pain, pressure, paresthesia, pallor, paralysis, pulselessness) yo konpare ak manm opoze a chak 1 a 2 èdtan apre yo mete plat oswa apre operasyon. Pou SKELETAL TRACTION, pwa yo pandye lib epi yo pa janm retire yo, yo netwaye sit pin yo dapre politik la, epi yo kenbe pasyan an aliyen.

COMPARTMENT SYNDROME is rising pressure inside a muscle compartment, within a cast or after a crush injury or burn, that cuts off perfusion. The earliest and most reliable signs are PAIN out of proportion to the injury that is not relieved by opioids and PAIN WITH PASSIVE STRETCH of the fingers or toes, followed by paresthesia and tense swelling; pallor, paralysis and loss of pulses are LATE and mean tissue is already dying. Act within hours: loosen or bivalve the cast or dressing, keep the limb at HEART LEVEL (elevating above the heart lowers arterial pressure into the compartment), do not apply ice, and notify the provider immediately; the treatment is a fasciotomy. FAT EMBOLISM syndrome follows long-bone and pelvic fractures, usually within 24 to 72 hours: sudden dyspnea, hypoxemia, confusion or restlessness, tachycardia, fever and a PETECHIAL rash over the chest, axilla and conjunctiva; treat with oxygen, support, and early immobilization of fractures to prevent it. Routine NEUROVASCULAR checks (the 6 Ps: pain, pressure, paresthesia, pallor, paralysis, pulselessness) are compared with the opposite limb every 1 to 2 hours after casting or surgery. For SKELETAL TRACTION, weights hang freely and are never removed, pin sites are cleaned per policy, and the client is kept in alignment.

🏥 Nan lavi reyèl

🚨 Sis èdtan apre yo mete yon frakti tibial nan plat, yon pasyan bay doulè a 10/10 malgre de dòz morphine epi li rele lè enfimyè a bouje dwèt pye l yo dousman. Enfimyè a pa bay yon twazyèm dòz epi li pa leve janm nan pi wo. Li dekole plat la dapre pwotokòl la, li kenbe janm nan nan nivo kè a epi li rele ekip òtopedik la. Fasciotomy sove janm nan. Doulè medikaman pa ka manyen se yon dyagnostik, se pa yon pwoblèm dòz.

🚨 Six hours after a tibial fracture is casted, a client rates the pain 10/10 despite two doses of morphine and screams when the nurse gently moves his toes. The nurse does not give a third dose and does not elevate the leg higher. She loosens the cast per protocol, keeps the leg at heart level and calls the orthopedic team. Fasciotomy saves the leg. Pain that medication cannot touch is a diagnosis, not a dosing problem.

📌 Sa pou retni

  • Doulè san soulajman ak doulè ak passive stretch bonè; pulselessness taUnrelieved pain and pain on passive stretch are early; pulselessness is late
  • Manm nan nivo kè a, plat la dekole, founisè a avize; fasciotomy trete lLimb at heart level, cast loosened, provider notified; fasciotomy treats it
  • Fat embolism: 24 a 72 èdtan apre frakti long-bone, petechiae sou pwatrin nanFat embolism: 24 to 72 hours after long-bone fractures, petechiae on the chest

📖 Tèm nan leson sa a

Sendwòm konpatiman (Compartment syndrome): Presyon k ap monte anndan yon konpatiman misk ki koupe sikilasyon san an; premye siy yo se doulè ki pa pwopòsyonèl ak doulè lè yo detire l pasivman.

Sendwòm anboli grès (Fat embolism syndrome): Grès ki soti nan yon frakti zo long ki rive nan poumon yo ak sèvo a nan 24 jiska 72 èdtan, ak dispne, konfizyon ak peteshi nan pwatrin lan.

✅ Teste tèt ou

Yon pasyan ki gen yon nouvo plat nan avanbra rapòte gwo doulè k ap ogmante analgesics pa soulaje ak doulè lè enfimyè a pwolonje dwèt yo pasivman. Ki aksyon priyorite enfimyè a?A client with a fresh forearm cast reports severe, increasing pain unrelieved by analgesics and pain when the nurse passively extends the fingers. What is the nurse's priority action?
  1. Bay yon dòz opioid anplisAdminister an additional dose of opioid
  2. Avize founisè a imedyatmanNotify the provider immediately
  3. Leve bra a pi wo pase kè a epi aplike glasElevate the arm above the heart and apply ice
  4. Dokimante epi re-evalye nan 2 èdtanDocument and reassess in 2 hours

Repons: B · Avize founisè a imedyatman

Sa yo se siy bonè compartment syndrome. Yo dwe soulaje presyon an nan kèk èdtan; leve pi wo pase kè a ak glas agrave perfusion an.

These are early signs of compartment syndrome. Pressure must be relieved within hours; elevation above the heart and ice worsen perfusion.

De jou apre yon frakti femur, yon pasyan vin konfi epi li kout souf, epi enfimyè a remake petechiae sou pwatrin nan. Ki konplikasyon enfimyè a ta dwe sispèk?Two days after a femur fracture, a client becomes confused and short of breath, and the nurse notes petechiae across the chest. What complication should the nurse suspect?
  1. Fat embolism syndrome
  2. Compartment syndrome
  3. Wound infection
  4. Osteomyelitis

Repons: A · Fat embolism syndrome

Dyspnea, eta mantal ki chanje ak petechiae sou pwatrin 24 a 72 èdtan apre yon frakti long-bone se klasik pou fat embolism. Compartment syndrome swiv frakti tou, men li lakòz doulè ak anfleman nan manm nan, pa konfizyon, dyspnea oswa petechiae sou pwatrin.

Dyspnea, altered mental status and chest petechiae 24 to 72 hours after a long-bone fracture are classic for fat embolism. Compartment syndrome also follows fractures, but it causes pain and swelling in the limb, not confusion, dyspnea or chest petechiae.

Ki rezilta ki yon siy TA compartment syndrome?Which finding is a LATE sign of compartment syndrome?
  1. Paresthesia
  2. Doulè ki pa pwopòsyonèl ak blesi aPain out of proportion to the injury
  3. Doulè ak passive stretchingPain with passive stretching
  4. Batman distal la absanAbsence of the distal pulse

Repons: D · Batman distal la absan

Pulselessness parèt sèlman apre ischemia pwolonje, lè domaj misk ak nè ka irevèsib. Paresthesia ka sanble ta paske se yon siy nè, men se yon rezilta bonè ki parèt byen anvan batman an pèdi.

Pulselessness appears only after prolonged ischemia, when muscle and nerve damage may be irreversible. Paresthesia can seem late because it is a nerve sign, but it is an early finding that appears well before the pulse is lost.
