Vwa Respiratwa, Respirasyon, Sikilasyon: Sa ki Tranche

Airway, Breathing, Circulation: The Tiebreaker

🫁 Kat pasyan, yon enfimyè, yon kesyon. Chak opsyon sanble ijan. Egzamen an pa vle ou santi ou prese. Li vle ou aplike yon règ nan yon segonn: ki moun ki pa ka respire?

🫁 Four clients, one nurse, one question. Every option sounds urgent. The exam does not want you to feel urgent. It wants you to apply one rule in one second: who cannot breathe?

💡 Ide prensipal

Vwa respiratwa, apre respirasyon, apre sikilasyon, ak de eksepsyon: are kadyak kay yon granmoun, yon timoun oswa yon tibebe se C-A-B (yon tibebe ki fèk fèt resevwa vantilasyon anvan), epi yon rezilta yo te prevwa ki stab pèdi devan yon chanjman inatandi.

Airway, then breathing, then circulation, with two exceptions: cardiac arrest in an adult, child or infant is C-A-B (a newborn at birth gets ventilation first), and an expected stable finding loses to an unexpected change.

🧠 Poukisa sa enpòtan

Lè opsyon yo konsène pasyan diferan oswa sistèm kò diferan, klase pa VWA RESPIRATWA anvan, RESPIRASYON an dezyèm, SIKILASYON an twazyèm. Yon vwa respiratwa ki bouche oswa ki an danje (stridor, anfle apre yon operasyon nan kou, yon reyaksyon anafilaktik, yon nivo konsyans ki bese ak vomisman) bat tout lòt bagay. Apre vini respirasyon: frekans respiratwa, efò, satirasyon oksijèn, pwoblèm tib pwatrin. Apre vini sikilasyon: senyen, doulè nan pwatrin, ipotansyon, aritmi. De prekosyon. Premyèman, yon granmoun, yon timoun oswa yon tibebe ki nan ARE KADYAK resevwa konpresyon anvan (C-A-B), paske san sikilasyon okenn lòt bagay pa enpòtan. Reanimasyon tibebe ki fèk fèt nan nesans se eksepsyon an: vantilasyon efikas vini anvan, epi yo ajoute konpresyon sèlman si frekans kadyak la rete anba 60/min malgre vantilasyon efikas. Dezyèmman, yon pwoblèm ABC yo TE PREVWA epi ki stab (yon pasyan ki gen COPD kwonik ak yon satirasyon abityèl 90%) ka pèdi devan yon chanjman inatandi kay yon lòt pasyan.

When options involve different clients or different body systems, rank by AIRWAY first, BREATHING second, CIRCULATION third. An obstructed or threatened airway (stridor, swelling after a neck surgery, an anaphylactic reaction, a decreased level of consciousness with vomiting) beats everything. Next comes breathing: respiratory rate, effort, oxygen saturation, chest tube problems. Then circulation: bleeding, chest pain, hypotension, arrhythmias. Two cautions. First, an adult, child or infant in CARDIAC ARREST gets compressions first (C-A-B), because without circulation nothing else matters. Newborn resuscitation at birth is the exception: effective ventilation comes first, and compressions are added only if the heart rate stays below 60/min despite effective ventilation. Second, an ABC problem that is EXPECTED and stable (a chronic COPD client at a usual saturation of 90%) can lose to an unexpected change in another client.

🏥 Nan lavi reyèl

🏥 Opsyon: yon pasyan de jou apre ranplasman anch ak doulè 6/10; yon pasyan ki fèk kòmanse gen stridor apre thyroidectomy; yon pasyan ki gen ponp IV li k ap sonnen; yon pasyan k ap mande ansèyman pou sòti lopital la. Stridor apre operasyon nan kou siyal yon vwa respiratwa k ap fèmen. Kòz yo gen ladan anfle, yon hematoma, laryngospasm ipokalsemik oswa blesi nan toude nè larenje rekiran yo. Doulè, ponp ak ansèyman, tout tann. Bon repons lan se sa ou ta kouri al jwenn nan, pa sa ou ta santi ou pi mal pou ou retade a.

🏥 Options: a client two days post-hip replacement with pain 6/10; a client with a new onset of stridor after thyroidectomy; a client whose IV pump is beeping; a client requesting discharge teaching. Stridor after neck surgery signals an airway that is closing. Causes include swelling, a hematoma, hypocalcemic laryngospasm or bilateral recurrent laryngeal nerve injury. Pain, pumps and teaching all wait. The right answer is the one you would run to, not the one you would feel worst about delaying.

📌 Sa pou retni

  • Vwa respiratwa bat respirasyon, respirasyon bat sikilasyonAirway beats breathing beats circulation
  • Stridor, anfle, anafilaksi ak yon nivo konsyans k ap bese se ijans vwa respiratwaStridor, swelling, anaphylaxis and a falling level of consciousness are airway emergencies
  • Are kadyak se eksepsyon an: konpresyon anvanCardiac arrest is the exception: compressions first

ABCs: Premye règ priyorite a: pwoblèm pasaj lè yo pase anvan pwoblèm respirasyon, ki pase anvan pwoblèm sikilasyon.

✅ Teste tèt ou

Ki pasyan enfimyè a ta dwe evalye anvan?Which client should the nurse assess first?
  1. Yon pasyan ki gen yon tous kwonik ak yon satirasyon oksijèn stab 91%A client with a chronic cough and a stable oxygen saturation of 91%
  2. Yon pasyan k ap rapòte gòj li sere ak yon vwa anwe apre yon piki myèlA client reporting throat tightness and a hoarse voice after a bee sting
  3. Yon pasyan ki gen yon glikoz nan san 180 mg/dL anvan manje midiA client with a blood glucose of 180 mg/dL before lunch
  4. Yon pasyan ki gen doulè ensizyon 3/10A client with 3/10 incisional pain

Throat tightness and hoarseness after a sting are early signs of airway swelling from anaphylaxis. Airway comes before all the others. The client with an oxygen saturation of 91% has a chronic, stable problem, so that low number is expected rather than a new threat.

Yo jwenn yon pasyan san pouls epi li pa respire. Lè ou sèvi ak gid reanimasyon aktyèl yo, ki premye aksyon an?A client is found pulseless and not breathing. Using current resuscitation guidance, what is the first action?
  1. Louvri vwa respiratwa a epi bay de soufOpen the airway and give two breaths
  2. Kòmanse konpresyon pwatrinBegin chest compressions
  3. Aplike oksijèn ak non-rebreatherApply oxygen by non-rebreather
  4. Tcheke tansyon anCheck the blood pressure

Repons: B · Kòmanse konpresyon pwatrin

Nan are kadyak sekans lan se C-A-B: konpresyon anvan pou retabli sikilasyon, apre vwa respiratwa ak respirasyon.

In cardiac arrest the sequence is C-A-B: compressions first to restore circulation, then airway and breathing.

Ki pasyan ki gen yon pwoblèm respirasyon yo ta dwe wè anvan?Which client with a breathing concern should be seen first?
  1. Yon pasyan ki gen COPD nan satirasyon baz abityèl 89% sou 2 L oksijènA client with COPD at the usual baseline saturation of 89% on 2 L of oxygen
  2. Yon pasyan ki gen opresyon ki gen wheezing li rete epi li twò soufle pou li paleA client with asthma whose wheezing has stopped and is too breathless to speak
  3. Yon pasyan ki ankye epi k ap respire 22 fwa pa minitA client who is anxious and breathing 22 times per minute
  4. Yon pasyan k ap mande yon incentive spirometerA client requesting an incentive spirometer

A 'silent chest' in asthma means air is no longer moving. It is a sign of impending respiratory failure, far more dangerous than loud wheezing. The client with COPD has a low saturation, but 89% on 2 L of oxygen is that client's usual baseline, so it is expected and stable.
