Living Wills, Proxies and the DNR That Is Not 'Do Not Treat'

📜 'DNR' does not mean 'do not care'. A client with a DNR order still gets antibiotics, pain control, oxygen and your full attention. The exam tests whether you know what the order actually stops.

Advance directives speak only when the client cannot. A DNR stops CPR and nothing else; every other treatment continues unless separately refused.
🧠 なぜ重要か
リビングウィルは、人工呼吸器や経管栄養など、自分で意思を伝えられなくなった場合に患者が希望する治療、または拒否する治療を記したものです。医療に関する持続的委任状（health care proxy：医療代理人）は、患者が自分で決定できなくなったときに代わりに決定する人を指名するものです。どちらも効力を持つのは患者が意思決定能力を失った場合に限られ、まだ自分で決定できる患者の意思は、いかなる文書よりも優先されます。DO-NOT-RESUSCITATE（蘇生措置拒否）指示は、患者の希望に基づく医師の指示で、心停止または呼吸停止の際にCPRを行わないとするものです。他の治療を差し控えるものではありません：苦痛緩和ケア、薬剤、酸素、吸引、手術はすべて継続できます。看護師は入院時に事前指示書について尋ね、写しを記録に保管し、家族の希望が判断能力のある患者の文書化された選択と対立する場合は患者を擁護します。
A LIVING WILL states what treatments a client wants or refuses if they cannot speak for themselves, such as ventilation or tube feeding. A DURABLE POWER OF ATTORNEY FOR HEALTH CARE (health care proxy) names a person to make decisions when the client cannot. Both take effect only when the client loses decision-making capacity; a client who can still make decisions overrides any document. A DO-NOT-RESUSCITATE order is a provider's order, based on the client's wishes, that withholds CPR in cardiac or respiratory arrest. It does NOT withhold other treatment: comfort care, medications, oxygen, suctioning and surgery can all continue. Nurses ask about advance directives on admission, place copies in the record, and advocate when a family's wishes conflict with a competent client's documented choices.

👨👩👧 A competent client with a DNR order develops pneumonia. The family says, 'He's DNR, so just keep him comfortable.' The client, alert and clear, asks for antibiotics. The nurse administers the ordered antibiotics. A DNR is not a refusal of treatment, and a capable client's voice beats the family's interpretation of a form.
