Delegation Cheat Code: Never Delegate What You EAT

👥 三つの文字が、試験の委譲問題のほとんどを振り分けます。それらを自分の担当として残し、それ以外のすべてのタスクは、患者の安定度と、任せる相手の業務範囲に合わせます。

👥 Three letters sort most delegation questions on the exam. Keep them for yourself, then match every other task to the stability of the client and the scope of the person you hand it to.

💡 要点

RNが保持するのは、Evaluate、Assess、Teach、看護判断、そして不安定な患者です。UAP：安定した患者のADLとバイタルサインです。LPN：安定して予測可能、ほとんどの薬剤（通常IVプッシュは不可）、指導の補強です。

RN keeps Evaluate, Assess, Teach, judgment and the unstable. UAP: ADLs and vitals on stable clients. LPN: stable, predictable, most meds (usually no IV push), reinforcing.

🧠 なぜ重要か

RNは、EATできることを決して委譲しません：EVALUATE（評価する）、ASSESS（アセスメントする）、TEACH（教える）です。看護判断と、不安定な患者に関わるあらゆることを加えたものが、RNの担当リストです。無資格の補助者（UAP）は、安定した患者の衛生ケア、食事介助、歩行、体位変換、水分出納の測定、バイタルサイン測定といった日常的で予測可能なタスクを行い、観察したことを報告します。LPNは、予測可能な転帰を持つ安定した患者をケアし、ほとんどの薬剤を投与し（IVプッシュ薬と輸血の開始は通常、州法や施設の方針で除外されます）、無菌ドレッシングを交換し、RNがすでに行った指導を補強します。決めるのはタスクの単純さではなく、患者の安定度と、任せる相手の業務範囲と能力です。

The RN never delegates what she can EAT: EVALUATE, ASSESS, TEACH. Add nursing judgment and anything involving an unstable client, and that is the RN's list. Unlicensed assistive personnel do routine, predictable tasks for stable clients: hygiene, feeding, walking, turning, intake and output, and vital signs on stable clients, then they report what they found. LPNs care for stable clients with predictable outcomes, give most medications (IV push medications and starting blood transfusions are usually excluded by state law or facility policy), change sterile dressings and reinforce teaching the RN already gave. Client stability, together with the delegatee's scope of practice and competence, decides, not the simplicity of the task.

📋 『新しい胸痛入院患者のバイタルサインを測って』は単純なタスクですが、不安定な患者に対するものなので、RNが担当します。『Mr. Leeをトイレまで歩行介助して』は単純なタスクですが、安定した患者に対するものなので、UAPに任せます。タスクは変わりませんでしたが、変わったのは患者です。

📋 'Take vital signs on the new chest-pain admission' is a simple task for an unstable client, so it stays with the RN. 'Walk Mr. Lee to the bathroom' is a simple task for a stable client, so it goes to the UAP. The task did not change; the client did.

Hygiene for a stable client is routine and predictable; the other three are evaluation, teaching and assessment. Evaluating a client's response to a new pain medication requires RN judgment and cannot be delegated to unlicensed assistive personnel.

LPNs care for stable clients with predictable outcomes; the others are unstable or require initial RN assessment or teaching. A client who needs the initial education about a new ostomy requires RN teaching and is not an appropriate assignment for an LPN.
