Premye Èdtan an: APGAR, Chalè, Vitamin K ak Sik
The First Hour: APGAR, Warmth, Vitamin K and Sugar
👶 Yon tibebe ki fenk fèt pèdi chalè nan kat fason nan premye minit lavi li yo, epi yon tibebe ki frèt boule glikoz ak oksijèn li pandan l ap eseye rete cho. Premye èdtan swen enfimyè a se sitou sou tèmodinamik.
👶 A newborn loses heat four ways in the first minutes of life, and a cold baby burns through its glucose and oxygen trying to stay warm. The first hour of nursing care is mostly about thermodynamics.
💡 Ide prensipal
Sèk, cho, po ak po. APGAR 7 a 10 nòmal. Vitamin K touswit apre nesans (nan premye 6 èdtan yo), plis pomad je. Lajònis anvan 24 èdtan patolojik; tcheke glikoz kay tibebe ki an risk yo.
Dry, warm, skin-to-skin. APGAR 7 to 10 normal. Vitamin K soon after birth (within the first 6 hours), plus eye ointment. Jaundice before 24 hours is pathologic; check glucose in at-risk babies.
🧠 Poukisa sa enpòtan
Nòt APGAR nan 1 ak 5 minit evalye rit kè, efò respiratwa, tonis misk, iritabilite reflèks ak koulè de 0 a 2 chak; 7 a 10 rasiran, 4 a 6 modere anòmal, epi 0 a 3 ba. Yo pa janm retade resisitasyon pou tann nòt la: lè yon tibebe bezwen li, li kòmanse anvan nòt 1 minit la. Siy vital tibebe ki fenk fèt yo atann: rit kè 110 a 160, respirasyon 30 a 60 (iregilye, ak ti poz kout anba 20 segonn), tanperati 36.5 a 37.5 °C. Anpeche PÈT CHALÈ lè ou seche li imedyatman, retire dra mouye yo, kontak po ak po, yon chapo, epi kenbe tibebe a lwen kouran lè ak sifas frèt (evaporasyon, kondiksyon, konveksyon, radyasyon). Bay VITAMIN K nan misk nan vastus lateralis pou anpeche senyen, pomad je ERYTHROMYCIN pou anpeche ophthalmia neonatorum, ak premye vaksen HEPATITIS B la: AAP rekòmande yon dòz nesans nan 24 èdtan pou tibebe ki stab medikalman ki peze 2,000 g oswa plis, tandiske depi fen 2025 orè CDC a kite dòz nesans la nan desizyon endividyèl lè manman an HBsAg-negatif; si manman an HBsAg-pozitif, yo bay vaksen an ak imunoglobulin hepatitis B nan 12 èdtan apre nesans la. Depiste tibebe ki an risk yo (ki gen manman dyabetik, ki gwo oswa piti pou dat yo, prematire, ki strese akòz frèt) pou IPOGLISEMI; tranbleman, letaji, move manje ak apne se siy, epi yon glikoz anba apeprè 40 a 45 mg/dL trete ak alètman bonè oswa dextrose IV. LAJÒNIS nan premye 24 èdtan yo PATOLOJIK (emoliz, enkonpatibilite ABO oswa Rh); lajònis apre 24 èdtan anjeneral fizyolojik. Fototerapi: pwoteksyon je yo mete, pati jenital yo kouvri, vire souvan, kenbe idratasyon epi siveye tanperati. Swen kòdon lonbrit la dwe sèk epi pwòp; li tonbe nan 1 a 2 semèn. Rezilta yo atann ki bay paran yo enkyetid: milia, congenital dermal melanocytosis (yo te konn rele li Mongolian spots: tach ble-gri ki pa danjere, yo dokimante yo pou yo pa konfonn yo ak ematom), acrocyanosis nan premye jou a, caput succedaneum ki travèse liy suture yo, ak reflèks souse, chèche, Moro, kenbe ak Babinski yo.
APGAR scores at 1 and 5 minutes rate heart rate, respiratory effort, muscle tone, reflex irritability and color from 0 to 2 each; 7 to 10 is reassuring, 4 to 6 is moderately abnormal, and 0 to 3 is low. Resuscitation is never delayed to wait for the score: when a baby needs it, it starts before the 1-minute score. Expected newborn vital signs: heart rate 110 to 160, respirations 30 to 60 (irregular, with brief pauses under 20 seconds), temperature 36.5 to 37.5 °C. Prevent HEAT LOSS by drying immediately, removing wet linen, skin-to-skin contact, a cap, and keeping the infant away from drafts and cold surfaces (evaporation, conduction, convection, radiation). Give VITAMIN K intramuscularly in the vastus lateralis to prevent bleeding, ERYTHROMYCIN eye ointment to prevent ophthalmia neonatorum, and the first HEPATITIS B vaccine: AAP recommends a birth dose within 24 hours for medically stable infants weighing 2,000 g or more, while since late 2025 the CDC schedule leaves the birth dose to individual decision-making when the mother is HBsAg-negative; if the mother is HBsAg-positive, the vaccine and hepatitis B immune globulin are given within 12 hours of birth. Screen at-risk infants (of diabetic mothers, large or small for dates, preterm, cold-stressed) for HYPOGLYCEMIA; jitteriness, lethargy, poor feeding and apnea are signs, and a glucose below about 40 to 45 mg/dL is treated with early feeding or IV dextrose. JAUNDICE in the first 24 hours is PATHOLOGIC (hemolysis, ABO or Rh incompatibility); jaundice after 24 hours is usually physiologic. Phototherapy: eye shields on, genitals covered, turn frequently, maintain hydration and watch temperature. Umbilical cord care is dry and clean; it falls off in 1 to 2 weeks. Expected findings that alarm parents: milia, congenital dermal melanocytosis (formerly called Mongolian spots: benign blue-gray patches, documented so they are not mistaken for bruises), acrocyanosis in the first day, caput succedaneum crossing suture lines, and the sucking, rooting, Moro, grasp and Babinski reflexes.
🏥 Nan lavi reyèl
🧢 Yon tibebe ki gwo pou dat li, ki gen yon manman dyabetik, gen tranbleman nan 2 èdtan lavi. Enfimyè a tcheke yon glikoz ak pike talon: 32 mg/dL. Paske tibebe a gen sentòm, enfimyè a avèti founisè a imedyatman epi prepare dextrose IV dapre pwotokòl la. Yo ka ofri li manje si tibebe a ka souse san danje, men sa pa dwe retade tretman IV la. Manje (oswa jèl dextrose) pou kont li se apwòch la pou glikoz ba san sentòm. Yo retcheke glikoz la apre tretman an. Iperglisemi manman an te fè ensilin fetis la wo, epi rezèv la te koupe nan nesans la. Egzamen an asosye 'tibebe yon manman dyabetik' ak 'tranbleman' epi li atann 'tcheke glikoz la' chak fwa.
🧢 A large-for-dates infant of a diabetic mother is jittery at 2 hours of age. The nurse checks a heel-stick glucose: 32 mg/dL. Because the baby has symptoms, the nurse notifies the provider immediately and prepares for IV dextrose per protocol. A feeding may be offered if the baby can suck safely, but it must not delay IV treatment. Feeding (or dextrose gel) alone is the approach for low glucose without symptoms. The glucose is rechecked after treatment. Maternal hyperglycemia drove fetal insulin high, and the supply was cut at birth. The exam pairs 'infant of a diabetic mother' with 'jittery' and expects 'check the glucose' every time.

📖 Tèm nan leson sa a
Nòt APGAR (APGAR score): Yon nòt tibebe nouvo fèt nan 1 ak 5 minit pou vitès kè, respirasyon, ton, refleks ak koulè; 7 jiska 10 nòmal.
APGAR (nòt tibebe nouvo fèt) (APGAR (newborn score)): Appearance (color), Pulse, Grimace (reflex irritability), Activity (muscle tone), Respiration (aparans (koulè), puls, grimas (iritabilite refleks), aktivite (ton misk), respirasyon): senk siy tibebe nouvo fèt yo bay nòt 0 jiska 2 chak nan 1 ak 5 minit.
Biliribin tibebe ki fenk fèt (Newborn bilirubin): Jònis fizyolojik parèt apre 24 èdtan epi li rive nan pik li ozanviwon 5 a 6 mg/dL nan jou 3 a 5; jònis nan premye 24 èdtan yo se patolojik, limit pou fototerapi yo swiv tablo AAP ki espesifik pou chak èdtan, epi nivo ki pre 20 a 25 riske kernicterus.
Glikoz nan san tibebe ki fenk fèt (Newborn blood glucose): Yon glikoz tibebe ki fenk fèt anba ozanviwon 40 a 45 mg/dL se ipoglisemi; tibebe manman dyabetik, ki gwo oswa piti pou dat yo, ki prematire oswa ki strese pa fredi yo depiste, epi tranbleman, move alètman oswa apne mande alètman oswa dextrose IV.
✅ Teste tèt ou
Yon tibebe ki fenk fèt gen yon rit kè 120, yon kri fò, bon fleksion, yon grimas lè yo aspire li epi yon kò woz ak men ak pye ble nan 1 minit. Ki nòt APGAR la?A newborn has a heart rate of 120, a strong cry, good flexion, a grimace to suction and a pink body with blue hands and feet at 1 minute. What is the APGAR score?
- 7
- 10
- 6
- 8
Repons: D · 8
Rit kè 2, efò respiratwa 2, tonis 2, iritabilite reflèks 1 (grimas), koulè 1 (acrocyanosis) = 8. 7 la soti lè yo bay men ak pye ble yo 0 pou koulè; sèlman yon tibebe ki konplètman ble oswa pal bay 0, kidonk acrocyanosis jwenn 1.
Heart rate 2, respiratory effort 2, tone 2, reflex irritability 1 (grimace), color 1 (acrocyanosis) = 8. 7 comes from scoring the blue hands and feet as 0 for color; only a completely blue or pale infant scores 0, so acrocyanosis earns 1.
Ki rezilta kay yon tibebe ki fenk fèt ki bezwen rapò imedya?Which newborn finding requires immediate reporting?
- Tach ble-gri sou sakwòm lanBlue-gray patches on the sacrum
- Po jòn ak blan je jòn yo remake nan 12 èdtan laviYellow skin and sclera noted at 12 hours of age
- Yon frekans respiratwa 48 ak ti poz kout 5 segonnA respiratory rate of 48 with brief 5-second pauses
- Ti papul blan sou nen anSmall white papules on the nose
Repons: B · Po jòn ak blan je jòn yo remake nan 12 èdtan lavi
Lajònis nan premye 24 èdtan yo patolojik epi li sijere emoliz; lòt rezilta yo se varyant nòmal. Yon frekans respiratwa 48 ak ti poz kout 5 segonn nan modèl tibebe ki fenk fèt yo atann epi li pa montre pwoblèm ijan lajònis bonè a montre a.
Jaundice within the first 24 hours is pathologic and suggests hemolysis; the other findings are normal variants. A respiratory rate of 48 with brief 5-second pauses is within the expected newborn pattern and does not signal the urgent problem that early jaundice does.
Poukisa enfimyè a seche yon tibebe ki fenk fèt imedyatman epi retire dra mouye yo?Why does the nurse dry a newborn immediately and remove wet blankets?
- Pou prepare pou enjeksyon vitamin K laTo prepare for the vitamin K injection
- Pou anpeche pèt chalè ak evaporasyonTo prevent heat loss by evaporation
- Pou amelyore nòt koulè APGAR laTo improve APGAR color scoring
- Pou stimile kriyeTo stimulate crying
Repons: B · Pou anpeche pèt chalè ak evaporasyon
Pèt chalè ak evaporasyon sou po mouye se pi gwo sous refwadisman nan premye minit yo; estrès frèt ogmante bezwen oksijèn ak glikoz. Seche stimile kriye ak respirasyon vre, men sa se yon benefis segondè; retire dra mouye yo sèvi sèlman pou kanpe pèt chalè a.
Evaporative heat loss from wet skin is the largest source of cooling in the first minutes; cold stress raises oxygen and glucose demands. Drying does stimulate crying and breathing, but that is a side benefit; removing the wet blankets serves only to stop heat loss.
