Ask the Question Directly
🕯️ A depressed client who has been withdrawn for weeks suddenly seems calm and gives his watch to a roommate. Most people read that as improvement. It can be the most dangerous day of the admission.
Ask directly, assess plan, means and intent, and treat sudden calm after depression as a warning. High risk means one-to-one observation and a safe environment.
ASK DIRECTLY: 'Are you thinking about killing yourself?' Asking does not plant the idea; it opens the door. Assess for a PLAN, the MEANS to carry it out, INTENT and a TIMELINE, previous attempts, and recent losses. Higher risk: a specific lethal plan with access to means, prior attempts, substance use, hopelessness, social isolation, recent discharge, and a sudden calm or energy after deep depression, which can mean the decision has been made. Giving away possessions and putting affairs in order are warning signs. The early weeks after starting an antidepressant can raise risk because energy returns before mood lifts. PRECAUTIONS: one-to-one observation for high risk, a safe environment with hazards removed per policy, frequent checks at irregular intervals, no sharps or cords, and medications swallowed under observation. Do not promise secrecy. So-called no-suicide contracts are not a substitute for assessment and supervision.
⌚ The nurse sits down with the newly calm client. 'You seem more at peace today. Sometimes people feel that way when they've decided to end their life. Are you thinking about suicide?' He says yes, with a plan for tonight. One-to-one observation begins and the provider is called. The question was uncomfortable. The alternative was a death that would have looked, in the chart, like recovery.
Ask directly; it does not increase risk
Plan, means, intent, timeline, prior attempts
Sudden calm, giving away possessions and early antidepressant treatment raise risk

A client with major depression says, 'Soon none of this will matter.' What is the nurse's best response?
Let's focus on something positive instead of this.'
Why would you say something like that right now?'
Things will get better soon, I promise you that.'
Are you thinking about killing yourself?'
Direct questioning is the standard. It allows assessment of plan, means and intent. Asking why the client would say that may seem to explore, but it puts the client on the defensive and never asks about suicide directly.
Which client has the HIGHEST suicide risk?
A client with a detailed plan, access to the means, and a prior attempt
A client who says, 'Sometimes I wish I could just sleep and not wake up'
A client who started an antidepressant last week and denies suicidal thoughts
A client who reports feeling hopeless but denies a plan or intent
All four clients carry some risk, but a specific plan with access to lethal means and a prior attempt are the strongest predictors. Hopelessness, passive death wishes and early antidepressant treatment all warrant assessment, but they rank below an active, specific plan with means.
A client admitted for suicidal ideation suddenly becomes cheerful and starts giving away belongings. How should the nurse interpret this?
The client has recovered and precautions can now be relaxed
The client may have decided to act and risk is increased
The client is manipulating staff to obtain an early discharge
The behavior is unrelated to risk
A sudden lift in mood with giving away possessions often signals that a decision has been made. Increase observation and reassess. Reading the lift in mood as recovery is the dangerous error; relaxing precautions now removes protection just as risk is rising.
