Never Ask Why: Therapeutic Communication That Passes
🗣️ A client says, 'I'm scared I'll die in surgery.' Four responses on the screen. Three are things a kind person would say. One is what a nurse says. The difference is the whole lesson.
Reflect the feeling and invite the client to say more. Avoid 'why', advice, false reassurance and changing the subject.
THERAPEUTIC techniques keep the focus on the client: OPEN-ENDED questions ('Tell me more about that'), REFLECTING feelings ('You sound frightened'), RESTATING, CLARIFYING, SILENCE, offering SELF ('I'll sit with you'), FOCUSING, and giving INFORMATION when asked. NON-THERAPEUTIC responses shut the client down: FALSE REASSURANCE ('Everything will be fine'), asking WHY, giving ADVICE or personal opinions, CHANGING THE SUBJECT, minimizing ('Lots of people feel that way'), judging, and defending the staff. Closed yes/no questions are acceptable for facts but not for exploring feelings. The exception is safety: when suicide or self-harm is possible, ask directly, for example 'Are you thinking about killing yourself?' Asking does not raise the risk. On the exam, the correct option usually names or reflects the feeling and invites more, and it almost never contains the words 'don't worry'.
💬 Client: 'I'm scared I'll die in surgery.' Option A: 'Don't worry, this surgeon is excellent.' Option B: 'Why do you think that?' Option C: 'Let's talk about your discharge plan.' Option D: 'You're afraid you might not survive the surgery. Tell me what's on your mind.' Only D names the feeling and opens the door. The other three are the exam's three most common traps in one place.
Open-ended, reflective, silent, present
Why' and 'don't worry' are almost always wrong
Facts can be closed questions; feelings need open ones, but always ask directly about suicide
A client says, 'I don't see the point in taking these medications anymore.' Which response is therapeutic?
Let me get you some water so you can take them now.'
Why would you say that?'
You have to take them or you'll end up back in the hospital.'
You sound discouraged about your treatment.'
Reflecting the feeling validates the client and invites exploration, which also allows screening for hopelessness. Asking 'Why would you say that?' seems to explore, but 'why' questions demand justification and tend to make the client defensive.
Which nurse response is an example of false reassurance?
Tell me more about your concern.'
I'll stay with you for a while.'
You seem worried about the results.'
Everything is going to be fine.'
Promising an outcome the nurse cannot guarantee dismisses the client's feelings and ends the conversation. 'I'll stay with you for a while' is offering self, a promise the nurse can actually keep, so it is not false reassurance.
A client sits silently after learning of a cancer diagnosis. What is the best nursing action?
Fill the silence with information about treatment options
Leave the room to give privacy
Ask the client to describe the five stages of grief
Sit quietly with the client and allow the silence
Silence and offering self are therapeutic; they give the client space to process and communicate presence. Leaving to give privacy can seem respectful, but it leaves the client alone with the news and can feel like abandonment.
